Introductions
a.
As a reminder, the information we will be
discussing for the purpose of housing referrals is confidential client
information. If you are not from an
agency on the GH CAN release, please do not take any copies of the client
information. If you do not need to bring
this information with you back to your agency, please return your copies to
Journey Home staff.
b.
If anyone has special events coming up on the
dates of any future CAN meetings (like a birthday) let Mollie know, and we can
bring celebration materials!

2.
Housing Referral Group
a.
Nine people have been housed since last week,
bringing our total number of chronically homeless individuals who have been
housed up to 75 since the start of the 100 Day Challenge!
b.
We provided new housing referrals to the
programs with openings, and reviewed all pending referrals in the GH CAN.
c.
One staff person had a question about the SOAR
process. SOAR is a process to help
people who are experiencing homelessness apply for Social Security
benefits. There will be a SOAR forum coming
up on Friday, September 11th at the Chrysalis Center- please contact
Matt Morgan at matt.morgan@journeyhomect.org
if you’re interested in attending.

Comprehensive Registry 10/1
a.
The Department of Housing has stated that all
Coordinated Access Networks in Connecticut need to have a comprehensive by-name
prioritized registry by 10/1. A lot of
our data collection in the past has relied on people coming through the CAN/211
process, and one thing we need to focus on for this deadline is ensuring that
all literally homeless households are assessed by the VI-SPDAT. We have estimated that about half of the
clients currently in shelter have not completed a VI-SPDAT.
i.
Preparation between today and September 20th
–
1.
We will review the current data collection
process at Journey Home
2.
All staff who have hard-copy VI-SPDATs currently
in their office need to start inputting them into HMIS. If staff are not immediately able to input
the VI-SPDAT assessment into HMIS, they should fax information about their
hardcopies to Mollie. If you need
assistance getting these VI-SPDATs into HMIS, have supervisory staff at your
agency contact Mollie.
3.
Each agency designated a point person for inputting
hard copy VI-SPDATs into the system.
ii.
Registry Week: 9/21 – 9/25
1.
One major concern is trying to reduce
duplication of efforts. One way to
reduce duplication is to enter VI-SPDATs into HMIS so staff can see if a
VI-SPDAT has been completed.
a.
Additionally, every day staff should fax
information about all VI-SPDATs completed on paper to Journey Home so that they
can compile a comprehensive list of who has already completed a VI-SPDAT each
day. Staff should reference this list
before doing a VI-SPDAT, to ensure that clients are not doing the assessment
multiple times.
2.
Some soup kitchens have been able to increase
capacity and host appointments during registry weeks, and some CAN assessment
sites are going to add additional drop-in hours during that week.
3.
Shelters who do not think they have enough capacity
to assess all households residing with them should contact Mollie at Journey Home
so that we can try to collaborate. A
couple of housing agencies indicated that they may be able to assist with
completing these assessments.
4.
We decided to have a short meeting during
registry week, focused mainly on the registry process.
4.
Transitional Living Programs –
i.
All Rapid ReHousing referrals and Permanent
Supportive Housing referrals are coming through some type of uniform
process. There will be a meeting soon to
discuss what the current process looks like with Transitional Living Programs,
to determine how we can ensure that we are not placing households who are chronically
homeless into TLPs, but are also able to fill openings in these programs
quickly.
5.
Announcements:
a.
A 90
minute VI-SPDAT training will be available for 8 people this Thursday, 9/10 at
Mercy Housing. Please email mollie.greenwood@journeyhomect.org if you would like to attend.
b.
Diversion
funding at CHR Enfield is available. If
you have a client at a CAN appointment that you believe is eligible for this
assistance, please contact Philomena McGee (860) 835-7283.
c.
Zero:
2016 Chronically Homeless Veteran Check-In –
i.
Andrea
knows there was one in Manchester that’s being worked with Shannon
ii.
Another
in the storage unit, he’s housed. He’s
in gpd now.
iii.
Philomena
– she sent someone to Amy – 75 years old homeless, may have called Amy. He was a vet in the 60-70s for 4 years.
d.
The
topic of a regional shelter bed waitlist will be discussed on 9/14 at 2:00 PM
at Journey Home, 241 Main St., Hartford, 5th floor.
e.
There
will be a SOAR Forum this Friday, 9/11 from 9 AM – 3 PM at Chrysalis Center,
255 Homestead Ave., Hartford.
f.
ImmaCare announced that they will have a
No-Freeze shelter again this year.
