GH CAN Leadership Meeting 6/2/2021

 

GREATER HARTFORD COORDINATED ACCESS NETWORK MEETING AGENDA

Wednesday, June 2nd, 2021 via Zoom

 1PM – 2:15PM

 

1.       Welcome and Introductions – 5 minutes

  1. Balance of State Continuum of Care Context/Updates – Lisa Quach/Crane Cesario – 10 minutes
    1. GH CAN Representatives for 2021

                                                               i.      Lisa and Crane are reps right now for GH, Crane is happy to step down if someone is interested.

                                                             ii.      Representative responsibilities – liaison btwn CT BOS and local CAN. Attend monthly meetings, on Fridays. Each CAN, can have 2 votes per CAN. Representatives take vote and present local CAN.

                                                           iii.      Please inform Crane and Lisa if you are interested in representing. Feel free to send an email to Sofina, Lisa, and Crane.  Once an email is sent, we can send out an email confirmation to take a vote.

    1. Review of GIW due by Thursday, June 3rd

3.       Length of Stay Data Updates – Lisa Quach – 10 minutes

a.       Discussed at operations as well. Yoshi lead the discussion.

b.      Discussed factors that increased length of stay – lot of it was COVID related

c.       3 main categories in GH – documentation barrier, motivation/ culture change, flexibility for obtaining documentation  

d.      HUD has a list of acceptable documents – send to leadership group. 

e.       Focusing on 5 folks who have been at shelter the longest and 5 folks who have been at the shelter the shortest

f.        Strategies have been identified to make shelter stay brief

g.       Will pull data again and continue conversations to see if any strategies were implemented

4.       COVID – 19 Agency Updates and ESG Updates – 20 minutes

a.       Updates/Vaccinations

                                                               i.      Capitol Region – on site, no more teleworking. Crane has been working with Mobile Crisis to have them come to present for leadership and ops. Mobile Crisis will be open longer on nights and weekends. Crane will look into new presenter once available. Capitol Region is partnering with police dept.

                                                             ii.      Triage at Immacare – 5 triage beds, no major issues yet. Clients sign a release notifying them the bed is temporary, clients are a little confused when they need to be transferred.

                                                           iii.      ARP Funds – proposed ARP allocation for homelessness service systems. Advocacy to correct funding. CCEH and Partnership met last week, increased budget, a total of 29million. More funding for staffing, shelters, etc. Finalized budget will be sent out soon.
