Hartford
100 Day Team – Updates
a.
100 day team, established a date and location
for Documentation Readiness Fair- we will be at Chrysalis May 8th. Chrysalis has offered to provide food and
volunteers in celebration of their 40 year anniversary.
i.
The team still needs to find copy paper,
copiers, laptops, volunteers, and staff from a number of agencies to help get
clients document ready.
ii.
Our leadership meeting will be April 1st at Sue Ann Shay Place
Apartments, 76 Pliny St., Hartford at 1:30.
iii.
The team received a portion of a script from
211 and are working together to determine modifications that we think could
improve appropriate appointments for clients and decrease our no-show rate.
iv.
We will work to figure out any statewide CAN
agenda items that the team would like to discuss with other regions.
3.
Statewide Police Reports- The Greater Hartford
Shelter Plus Care program staff can get statewide Police Reports for clients
who need them. To request a report,
please email Crane Cesario (crane.cesario@ct.gov)
to let her know you are sending a request and include your contact
information. Then fax her with client
name, any aliases, DOB, SSN and signed release of information. The fax number is 860-297-0930.

Days – Greater Hartford’s Advocacy Days time is April 1st from
8:30 AM – 10:00 AM.

and there was a very enthusiastic group that has committed to attending.

HUD
Updates – HUD Policy Brief on Coordinated Entry and HUD Notice on
Appropriate Placement for Transgender Persons in Single-Sex Emergency Shelters.
a.
The Policy Brief on Coordinated Entry was
closely aligned with what we have been planning in our region.
b.
The Notice on Appropriate Placement for
Transgender Persons in Single Sex Emergency Shelters provided some updates.
i.
In the past, the state of Connecticut has had
more stringent protections and nondiscrimination policies than Fair Housing or
the federal agencies. The state of
Connecticut has mandated nondiscrimination on gender identity or expression.
ii.
HUD’s new notice requires that single-sex
shelters rely on client self-report of gender identity rather than any other
indicators. Providers are prohibited
from asking about medical procedures or documentation to verify their sex, or
invasive questions about anatomy. Client
report is what counts.
iii.
At a minimum, this notice requires that
transgender clients are allowed to shower at a different time.
iv.
Best practice is to have a conversation with
the client about what they feel is important to their safety and security while
in the shelter.
7.
Department of Housing Shelter Bids
a.
Last week, the Department of Housing
announced shelter grants for the 2015-2016 fiscal year. Two of our regional shelters, Tri-Town
Shelter in Vernon, and The Open Hearth Shelter in Hartford, did not receive
funding from the DOH. It is not yet
clear what that will mean for our region.
The new funding will come into effect on July 1, 2015.
8.
Priority Lists for the 100 Day
Campaign
a.
Journey Home has been working to
compile a master list of clients in the region who have interacted with our
shelter in the past few months. This
list is a combination of data from HMIS and the Greater Hartford Universal
Housing Application. The list indicates
which clients have completed a Universal Housing Application, which clients
have had a CAN assessment, and which clients have self-reported as chronic.
i.
In addition, Journey Home has used
HMIS shelter records to try and ascertain which clients already have
documentation of chronic homeless status according to their records. These clients have been indicated as having
verification of chronic status.
b.
Four lists were distributed at
today’s meeting.
i.
One list contains all clients who
have HMIS verification of chronic homeless status. This does not mean that they have completed a
VI-SPDAT, UHA, or that they have all necessary documentation to enter a housing
program.
ii.
Another list is contained all clients
who had self-reported that they were chronic.
We hope that providers will help us look through this list and identify
clients that they also believe are chronically homeless.
iii.
The other two lists included clients
staying at the airport during PIT, and clients who were unsheltered during
PIT. Although these clients aren’t
necessarily documented as chronic, they need specific focus because they are
not engaging with shelters.
c.
Now that we have begun to identify
priority clients for housing, we need to make sure as many of them as possible
are document ready to be matched with housing.

The group reviewed the list of prioritized
clients.

Hartford
100 Day Team:
i.
Hartford 100 Priority List- How can the team
support this list, and what are our next steps?
1. How
can we use this list to start using coordinated navigation?
2. Amy
Robinson volunteered to be a point person for navigation for the clients on the
prioritized list.
3. Chrysalis
CABHI staff can also help with navigation and document readiness for any
clients that are prioritized.
4. The
team discussed a number of clients on the list- there are some concerns that
some especially vulnerable clients have VI-SPDATs are not reflective of our
needs. For those clients, staff should
submit an assessment concern form to Journey Home so that we can discuss them.
ii.
Document Fair – Establish location, determine
next steps
1. The
team has decided to host this event at Chrysalis Center, 255 Homestead Ave,
Hartford on May 8th. We hope
to host the event between 9:00 AM and 3:00 PM.
2. The
team is working to plan transportation to and from this event for clients. Chrysalis may have a couple of vans we can
use.
3. We
need to contact the DMV for photo IDs, City of Hartford for Hartford birth
certificates, and SSA for Social Security cards as well as disability
information. We also want to reach out
to DSS. We also want to reach out to Vital Records for out of town birth
certificates.
4. Puerto
Rican birth certificates may need a specific staff person designated to help
fill out the application.
5. InterCommunity
had previously volunteered to help with disability verification.
6. By
the end of this week we hope to have drafted a flyer for the documentation
fair- Heather will
iii.
2-1-1 Script Review – What language would we
like to insert / modify?
1. Mollie
will be collecting the edits that we have made and drafting a modified script
that we would like 211 to test out for clients in our region.
iv.
Designate point person for each task or goal.
1. Since
Nate Fox from Center Church was already planning to participate in advocacy
days, we determined that he could also serve as the Harford 100 Team
representative at that event.
2. Amy
Robinson has volunteered to be a point person for navigation for these
clients.
3. Jose
Vega has volunteered to use his contacts to recruit agencies for the document
fair.
