Young Adult Services Presentation – Seweryn Borucki
a.
Young
Adult Services through DMHAS is a program for young adults age 18-25. The level of care provided is just under
inpatient hospitalization. To be
eligible for the program, these youth must demonstrate SMI. It is a voluntary program. About ½ of the program participants are
through DCF, the other half are community referrals. There are a number of therapists, and each
team has 5-6 case managers DMHAS is a
treater of last resort, and tries to consistently refer to community based
mental health services.

Coordinated Entry

Cold Weather Updates – Sarah DiMaio
i. The
warming center continues to be at capacity most nights.
ii. Shelters
should continue taking referrals from the Warming Center to fill available
beds.

Point In Time Count Updates – Cat Damato and
Crane Cesario
i. Training
will be held tomorrow at CRT from 5:00PM-7:00PM. Training for the Youth Count will be held on
Monday from 5-6PM at My Sisters’ Place

Changes to the Housing Matching Schedule – Beginning
in February the Rapid ReHousing Matching meeting will merge with the
Individuals Coordinated Exit meeting so that there will be one monthly meeting
for all housing resources for individuals.

CT Department of Housing has made Capital
Funding available for shelter improvements.

Beginning in January this committee’s meeting
schedule will be changing to meet on the first and third week of each
month. Check your email for an updated
link for the CAN calendar.
c.
As of January 1, 2018 Logisticare will no longer
be providing non-emergency medical transportation (NEMT) in CT. A new company has the DSS contract now: VEYO,
a part of Total Transit Company. To
request rides for medical appointments that will take place on or after January
1, 2018, the new telephone number to call is: 1-85-478-7350. Clients must still request rides 48 hours in
advance. VETO has established a local
call center that is available with 24/7/365 access for trips on or after
January 1, 2018. The call center number
is 1-855-478-7350.
