No staff had case conferences to present this
week.

Coordinated Entry
a.
Cold Weather Planning – Sarah DiMaio
i. Warming
Center/Cold Weather Updates – The Warming Center and Triage Centers will both
be opening next Thursday, December 7th. Over the upcoming weekend, Salvation Army
Marshall House will be doing triage for folks who present directly or who are
calling 211 after hours.
ii. There were
some unexpected issues with the space when Salvation Army got to do the
walkthrough with the City of Hartford at the Willie Ware Rec Center. There were still a number of events that had
reserved the space for the coming months, and there is also community
programming happening until 7PM many nights.
As a result of this programming, the Warming Center at Willie Ware will
not open until 7PM each night. There was
no internet yet at the rec center, as the RFP had stated, so the staff will
utilize hot spots when the site opens.
iii. Marketing
Materials – Salvation Army distributed a flyer that sites could begin posting,
with an opening date of December 7th.
iv. CAN Staff
Protocols – protocols for all GH CAN staff were distributed, and will be shared
via email along with the flyer.

Point In Time Count – Cat Damato
a.
PIT maps – Cat Damato – the Point In Time
count will be taking place on the evening of January 23rd with the
Unsheltered Point In Time count taking place the morning of Wednesday, January
24th. Cat Damato and Crane
Cesario will be coordinating the count for the Hartford sub-COC and the Greater
Hartford sub-COC, with CHR organizing the unsheltered count in Manchester. Cat brought PIT maps and requested that any
staff who know of places where folks may be staying should check these maps
out, and leave a label on locations where folks are known to be
unsheltered. This information will help
CCEH identify block groups where people will go out looking on the morning of
the count. If you’re interested in
volunteering, contact Cat Damato.
b.
Youth Count Updates – Steve Hurley – this year’s
Youth Count will be a survey that will be administered all throughout the Greater
Hartford area with the hopes of identifying youth who are homeless or unstably
housed. The survey is administered on a
tablet or a smartphone, and takes between 5-10 minutes. If you’re interested in volunteering, contact
Steve Hurley.
i. Youth in
Emergency Shelters and Youth Count – Rochelle Currie – any young adults who are
currently in emergency shelter should participate in this survey. If your organization needs additional volunteers,
please contact Steve Hurley.

New
Requirement: Timestamp for Enrollments on Homeless Verifications – Crane
Cesario – Moving forward, if an HMIS Enrollment Page is being used to
demonstrate current homelessness, that page needs to have a date
indicator. For example, if a shelter
enrollment begins on a past date and goes through “present” it’s important that
the form indicates what today’s date is.
A timestamp used when the sheet is being printed would be
sufficient.

Leadership Updates – Mollie Greenwood
a.
Beginning in February, we are hoping to start
streamlining some of the housing matching meetings, with the overall goal of
combining Coordinated Exit for Individuals (Tuesday’s housing matching meeting
for chronically homeless households) with the Individuals RRH Matching meeting.
i. There were
concerns around overall time of the matching meeting. There are times when both of those meetings
can run long, and right now there are some staff who attend RRH for individuals
and the following family matching meeting.
ii. There were
also concerns around the time of the current Tuesday meeting, as it presently
runs around lunchtime.

being a veteran, please call CRT’s Supportive Services for Veteran Families
(for both families and individuals)
b.
World AIDS Day will be taking place on Friday,
December 1st, 2017 from 6:30-8:30 at the Wadsworth Atheneum.
c.
Security Deposit Guarantee Program is only
available for chronically homeless clients at this time.
d.
The next Cold Weather Planning meeting will take
place on Mondays at 3:00 PM at Journey Home.
