Case Conferences – Fred Faulkner
a. Cornerstone
shelter brought a client up for discussion, determination was that Cornerstone
staff would contact Atlas Behavioral Health and request on-site behavioral
health services for this household.
c. Other
cases?
a. Lisa says she has a client who has voucher with
eviction and with year old child who has a wheel chair.
b. Lisa is looking to find landlord that will take
voucher and has wheel chair accessibility.
c. Amy says Hart Property would work with them but may
not have wheelchair.

City of Hartford Municipal IDs (see handouts) –
Janice Castle
a. Janice says the
community engagement department has access to the Lariby Fund and is only for
back rent and limit is $1,000. They also have access to five pieces of
furniture for free and only have to be Hartford resident. Not sure if furniture
can be delivered.
b. Janice says the
municipal ID program and will have final brochure available by Friday.
c. Residents must
be able to prove residency to be eligible.
d. Records go to
vendor and not to city of Hartford. Therefore, there are no records kept when
applying for ID.
e. Client can get a municipal ID from city of Hartford if they are current residents.
f. If card cannot
get delivered they will be delivered back to the community engagement
department office.
g. Janice asks if
there are any other languages need to be available in other languages to
contact her.
h. Janice says
program will be open on Friday.

Jose asks if they can waive the fee on the homeless population
ii.
Janice says no they can’t waive the fee.
iii.
Wendy asks if this ID can be used to get access to other vidal documents.
iv. Janice says that is a working process, but the
Police Department ad DSS can accept the ID.
v. Amy suggests to send copy of brochure to Mollie and
she can send that information through e mail.

Roger suggests to add Polish translation for form.
i. Lisa asks if
they will be reaching out to local check providers.
k. Janice says
they are waiting for them to get back to them.
l. Wendy asks if
ID could help clients get phones.
m. Janice says she
can look into that.

Updates from CAN Leadership – Mollie Greenwood
a. Crane introduced some of the work that she and Matt have been doing to analyze the family homelessness data in GH CAN. As a community, as we try to figure out how to reconfigure our resources, we may need to ask questions like "How can we reduce length of stay in family shelters to move families in need of shelter in more quickly?" She and Matt will continue working on this data.
b. Crane says CAN
might think about reconfiguring the available shelter beds.
i. Crane says that according to HUD PSH programs now needs
verification of homelessness 24 hours before receiving certificate.
ii.
Crane says we need to make sure this needs to be communicated to the CAN.
iii.
Heather says creating a template internally at each will make this process easier. At South Park Inn, shelter staff have been used to generating homeless verification letters, and having a template for each shelter may make this process easier to sustain.
c. Mollie says
Theresa has a connection with a furniture moving provider that would cost 75 dollars
an hour.
d. Mollie says
that utilities assistance may be available from Hands on Hartford in the
upcoming months, which may help reduce one of the barriers to housing we have discussed lately.
e. Mollie says the
Rapid Rehousing referral process has been a challenge in the past few weeks. Right now, CAN leadership is working on a list of questions for case managers to help determine about who may be a good candidate for rapid re
housing.
f. Mollie requested that for
next CAN Operations agencies have waitlist users to attend the next meeting because the subject will be about improving the shelter waitlist.

Announcements
a. The
CT Coalition to End Homelessness will be hosting their Annual Training
Institute on Thursday, May 18th. Register online at www.cceh.org.
b. There
are openings available in this month’s full-day CAN Training on Friday,
5/19. Email mollie.greenwood@journeyhomect.org
to register.
c. Dinner
in the Park will take place Saturday, May 27 at Bushnell Park at 5:00 PM.
d. The
Section 8 Housing Choice Voucher Preference is open again.
