3.      Community Support Program Presentations
a.      Chrysalis Center – Amber Freeman
a)      Amber says Community Support Program is a skilled building case management program and the goal is to get clients to live self-sufficiently.
b)     She says the client must have a mental health diagnosis and actively be seeing a clinician.
c)      The program will help clients with community supports to help them live independently. CSP staff hosts groups, refers out to other providers in the community, and works with existing therapy supports to help the household.
d)     The referral process for their program has to be referred by a clinician and must have completed a recent bio-psycho social evaluation to see if client is right fit for program. The therapist can complete the CSP application and fax it over to Amber at Chrysalis. She generally has a couple of weeks to review the document but if staff send her an email to notify her of the referral, she will prioritize looking at it.
e)     Amber Higgins asks if there is a waitlist.
a.      Amber says there is currently no waitlist.
b.      Capitol Region Mental Health Center – David Susanin
a)      David says in order to for clients to get services they have to be a client of DHMAS Capital Region in Hartford, West Hartford, and Avon.
b)     David says clients have to be enrolled in DHMAS Capital Region. If clients are not currently clients on CRMHC, the CSP team will try to refer the household to one of the other CSP providers in the community.
c)      David says their Community Support Program tries to get clients into the program in a day or two after they have an assessment.
d)      Clients must have mental health or substance abuse to eligible regardless of income and insurance.
e)     The program also takes clients who cycle in and out of hospitals.
f)       David says they help clients fill out housing applications and help them find clothes and sometimes offer security deposits.
g)      David says their program they try to help clients to live independently.
c.      InterCommunity – Shawn Decker
a)      Shawn says their Community Support Program helps clients to live independently. The client must have a major mental health diagnosis in order to be eligible. Unlike the two other Community Support programs, households do not need to already be connected to ongoing clinical services to be eligible for this program. Households who are seeking CSP services can go to InterCommunity’s walk-in hours to get an assessment for CSP.
b)     Shawn says they encourage clients to go to group sessions before being assigned to a case manager.
c)      Shawn says if clients have a section 8 voucher they will help the client find an apartment.
d)     The waitlist for the program very from month to month, but the average waitlist time is six to eight weeks.
e)     Shawn says they have a soft catchment area which is East Hartford, Glastonbury, Rocky Hill, Marlborough, Hartford, Manchester, and New Britain.
4.      Leadership Updates – Mollie Greenwood
a)      Mollie says Salvation Army Marshall House will be opening a No Freeze Shelter for individual women and families on November 1st.
b)     Cat says she will be the Hartford COC at part time capacity and will be the lead for the PIT Count.

a)      Mollie says there are only 5 verified chronic not yet matched to housing.
6.      Announcements
a.      Hands On Hartford and a number of community partners are working on planning the Day of Sharing and Caring, and needs lots of help planning and pulling off the event.  If you’re interested in assisting, please contact swalker@handsonhartford.org.  The event will take place on 11/2/17
b.      Salvation Army is offering an Anti-Human Trafficking Training at Capitol Region Mental Health Center Tuesday, 9/26 from 1-4 PM.  Register for training with Cat Damato at damatoc@crtct.org
c.      Security Deposit Guarantee Program is only available for chronically homeless clients at this time. 
d.      The next Cold Weather Planning meeting will take place on Monday, September 25th at 3:00 PM at Journey Home
