Coordinated Entry
a.
ImmaCare Renovations – Rebekah Lyas
ImmaCare will close during renovations and staff will be seeking
alternate options for clients (Rapid Exit, safe double-up, etc) and ImmaCare
staff will do a housing purge. No longer accepting new clients as of May 10,
2019. Hopes are that Immacare shelter case managers will be able to assist with
diversion, outreach, and any other areas there may be a need.

YHDP Navigator Enrollment Questions Request –

i. Case
Managers at Adult Shelters should complete this set of questions with any young
adult in their shelter, and should upload it to files and documents in CT HMIS
once completed, and notify the appropriate navigator. If shelter case managers don’t know who the
appropriate navigator is they should contact Journey Home.

CT HMIS: Duplicate Creation – Crane Cesario,
talk about it more in 2 weeks.

Approval of Additional Homeless Outreach

Stephanie Boyce has been approved by the GH CAN, and will be given
outreach/navigator privileges.
