3.      Coordinated Entry

a.      ImmaCare Renovations – Rebekah Lyas
ImmaCare will close during renovations and staff will be seeking alternate options for clients (Rapid Exit, safe double-up, etc) and ImmaCare staff will do a housing purge. No longer accepting new clients as of May 10, 2019. Hopes are that Immacare shelter case managers will be able to assist with diversion, outreach, and any other areas there may be a need.

b.      YHDP Navigator Enrollment Questions Request – Mollie Greenwood
                                                    i.     Case Managers at Adult Shelters should complete this set of questions with any young adult in their shelter, and should upload it to files and documents in CT HMIS once completed, and notify the appropriate navigator.  If shelter case managers don’t know who the appropriate navigator is they should contact Journey Home.

c.      CT HMIS: Duplicate Creation – Crane Cesario, talk about it more in 2 weeks.

d.      Approval of Additional Homeless Outreach Worker for SmartSheets – Alexis Gaynor

Stephanie Boyce has been approved by the GH CAN, and will be given outreach/navigator privileges.

4.      Coordinated Exit:
a.      Reminder: Clients in Jeopardy of Losing Housing should be submitted for 5/15 Operations Committee

a.      Rapid Exit Funding IS AVAILABLE.  Contact Mary.McGowan@journeyhomect.org with any questions!
b.      Family Shelter Check-In will be taking place immediately after today’s meeting from 3:30-4:00PM
c.      Data around housing and our shelter priority list is available on p. 2
d.      CCEH has funding available for victims of Hurricane Maria- if you are working with anyone who was a hurricane evacuee please contact Mollie Greenwood or Lisa Quach immediately.
e.      CCEH has funding available for childcare for families in shelter!
f.       My Sisters’ Place Security Deposit Funding is available – contact person is Darryl Gillus at 860-969-1906
