1.  Welcome and Introductions

2.  Clients in Jeopardy of Losing Housing
       PB #214040/221447 - Itzel (Chrysalis absent)
  • One person at risk of losing housing is getting certificate back to find housing; Per Crane, John Mack will get his certificate back after DMHAS reviewed case. 

3.  Coordinated Entry
a.  Proposed Shelter Waitlist Wipe (Starting fresh for warming center)
  • Proposed plan to wipe waitlist for shelters and start new when the warming center opens December 1st
  • The goal is to wipe SmartSheets before December 1st to start with a new list
  • Some questions and concerns from the team were:
    • What happens if we accidently remove those who are verified homeless?
    • Can the list be archived before purging?
    • When will Diversion staff stop adding clients to waitlist? 
    • Will the Diversion staff receive advance notification? - Diversion team will be notified far in advance before they can stop adding to SmartSheets. Leadership will be meeting with the Diversion staff to clarify roles.
  • A vote was taken by the Operations Team that determined that the list can be wiped but we must archive the current waitlist
b.  Refusal of shelter due to safety reasons
          i. Protocol (Remove or not)
  • If a client expresses that they can not go to a certain shelter because they feel unsafe CAN should be selecting “Refuse - Safety/Special Reason” in SmartSheets. They will be considered for the next shelter.
         ii. Documentation in Smartsheets on shelter waitlist
  • CAN can select “Refuse - Safety/Special Reason” in SmartSheets and include in the notes column what the exact reason is, examples can be that the area is a trigger, there is someone who stays at the shelter that they may have problems with, etc. 

4.  Cold Weather Updates – Sarah and/or Kelly
  • Warming Center in Hartford is expected to open on December 1st through March 31st
  • Salvation Army is in the process of hiring part time staff for the Warming Center, they are expecting for trainings for new hires to start in November
  • Salvation Army has reached out to seven churches in Hartford to have them host families and single women one night per week for larger capacity, so far three churches have been in contact with the Salvation Army to be possible hosts.
  • The City of Hartford would like to designate the Warming Center for Hartford residents. 
  • Warming Center will have onsite CAN appointments available for clients to divert them if possible.

5.  Leadership Updates
  • Strength focused trainings to occur soon.
  • Point In Time (PIT) planning in progress, PIT team is looking for volunteers.
  • For the first time Central CAN will be doing the PIT count in the morning like Greater Hartford from 5am to 7am. 


 6.  Youth Updates 

  • YHDP is implementing a new program with HUD and DOH to help youth couples and individuals under the age of 25
  • Navigators can work with youth and CCEH
  • There are a total of 16 youth beds (12 at the Connection, 4 at Salvation Army)
  • Youth cannot be on shelter waitlist until they are verified as unsafe or doubled up/unsafe.
