GH CAN Leadership / GH CAN Sub-COC 1/16/2019

1.      Sub-COC Items:
a.      Point In Time Count – Zoe Schwartz, Crane Cesario
                                                    i.     Zoe brought waivers to the meeting.  We’re in the midst of planning, and we’re still seeking volunteers.  Particularly in the City of Hartford we’re still in need of volunteers.
b.      Save The Date 3/21/19 for CT BOS Training: Fiscal Components of Managing a COC Grant
                                                    i.     This training seems like it may be introductory for managing COC grants.
                                                   ii.     The CT BOS meeting is this Friday at Middletown at the State Library.  Carpool if you can! 
c.      CT HMIS Case Notes for Diversion: Stephanie Corbin, Sarah DiMiao, Jen Greer
                                                    i.     Anyone who doesn’t have CAN level access to CT HMIS isn’t seeing the case notes from 211 or the Diversion Center. 
                                                   ii.     Crane and Lisa will discuss putting in a request in the release bin for outreach, diversion, CAN coordination, and shelters need access. 
                                                  iii.     With changes happening at CCEH, it may make sense for Lisa to talk with Brian. 
                                                  iv.     Is this for everyone going through, or just certain circumstances of folks coming through the Diversion Center. 
                                                   v.     Stephanie and Lisa and Crane will connect on this item.  And we can revisit this. 


2.      Coordinated Entry:
a.      Cold Weather Protocol Updates – Sarah DiMaio
                                                     i.     Hit capacity for Warming Center.  Had 51 folks in there this past weekend.  Everything has been running smoothly.  We’re getting crowded and busy.  Shelter transfers are happening.  The biggest challenge right now is that we have a decent cohort of folks who are refusing shelter beds.  10-15 people are refusing shelter beds up front. 
1.      There’s a lot of active substance use and a lot of untreated mental health.  There are clinicians and harm reduction specialists at the warming center each day. 
                                                   ii.     Warming Center doesn’t have the capacity to follow up with each instance of someone saying that they got discharged. 
                                                  iii.     Some folks are doing month-to-month rooms for rent.  Some folks do seasonal work and when their paycheck drops off they come to the Warming Center. 
                                                  iv.     There are a few folks who had housing who were uncomfortable with individual apartments who are coming to the Warming Center. 
                                                   v.     Lisa will ask the Enfield Warming Center if transfers would be acceptable.
                                                  vi.     Families aren’t coming into the overflow spaces.  The only women at the Warming Center are those refusing beds. 
                                                 vii.     If we want to set our own local threshold on Cold Weather Activation, we can do that. 
                                               viii.     Mollie to find the Cold Weather Activation from back when Nate and Christie were running Cold weather.
b.      Shelter Priority List – Stephanie Corbin
                                                     i.     We have largely eliminated a shelter waitlist for families. 
                                                   ii.     Stephanie would like to propose eliminating a waitlist across the board.  Our outreach teams aren’t verifying folks as sleeping outside. 
                                                  iii.     She’d like to propose not using it during the winter and just directing people to the Warming Center.  The only way to access shelter beds is to present at a Warming Center. 
                                                  iv.     The only way to access a shelter bed is to be referred from the Warming Center or verified by an outreach worker. 
                                                   v.     Staff are not allowed to turn people away from the Warming Center to get extra staff to the building.  
                                                  vi.     Leadership approved stopping waitlist additions.
                                                 vii.     There are probably a lot of folks who can get removed. 
                                               viii.                    Sarah and Kelly will highlight anyone who is marked as “no freeze” program.  Stephanie and Kelly and Sarah will clean the list up. 
                                                  ix.     Mollie will let 211 know.               
c.      Homeless Outreach Capacity – Matt Morgan
                                                     i.     There’s some data in  your packet that’s called GH CAN Outreach Data. 
                                                   ii.     Statewide we’re looking at homeless outreach capacity. 
                                                  iii.     Our regional homeless outreach teams could only verify 152/689 folks referred for an outreach worker.  161 couldn’t be reached.  132 were found and were not unsheltered. 
1.      Crane requested this be combined onto one page and figure out what the outliers were. 
                                                  iv.     We have under 3 folks.
                                                   v.     We’re requesting additional capacicty of 3FTE plus .25 supervision.

1.      We have some CH verified folks who are refusing housing opportunities.
