2.      Leadership Updates – Crane Cesario

a.      We broke up to discuss long stayers and talk about how our system can respond to this community need. 
                                                    i.     One big idea is that we can’t continue to do more with less.
                                                   ii.     We also thought about maybe having some folks who were recently housed do some workshops in the shelters on what their experience getting housed was like. 
                                                  iii.     We are going to support a human trafficking letter – nobody in our area uses that type of funding right now.
                                                  iv.     The COC is doing their work – we’re losing Zoe for another position. 
                                                   v.     Advocacy Day is coming up.  In the past, some programs have LOST funding because nobody from their program showed up.  A lot of executive directors that we’ve asked cannot be there.  It’s Wednesday April 17th. 

3.      Case Conferences – Fred Faulkner
We returned to our previous discussions around long-stayers in shelter.
4.      Coordinated Exit:
a.      One suggestion from the last meeting is to incentivize shared housing by having a shared housing mixer. 
                                                    i.     There’s a ready-to-roommate session.  What if we did like once a month a mixer, where clients are in control of their own programs? 
1.      Can we generate incentives, like rapid exit covering two months of funding.  
2.      Add this to the next agenda for more discussion.

b.      Case Conferencing Households in Jeopardy of Losing Housing – Lisa Quach, Crane Cesario
                                                    i.     We had this really good idea to discuss people in jeopardy of losing housing.  The time that we’re spending talking about them is expanding and is eating into time at the Individual Solutions meetings.
                                                   ii.     Kara suggested that everyone needs to ensure that agencies discuss people in jeopardy of losing housing internally.  One suggestion is to have some time here to discuss this. 
                                                  iii.     We need to think about how we want to manage the volume of work we’re doing. 
1.      Tahaira suggested not bringing up clients for whom the agency has a plan in place.  Maybe the expectation is there needs to be some internal discussion first.  And presentations need to come with a strengths based focus. 
2.      There should be an emphasis to identify what’s next, and have the CAN be more of a reviewing.
3.      To Lisa’s point we need to take a breath after that one.  It’s a repeated cycle of what we’re doing. 
4.      There’s a point of becoming so self-aware that we don’t want to do anything wrong. 
a.      CSH standards now require that programs bring folks to the CAN in advance.
5.      We may just need supportive housing providers to come together to discuss
6.      There’s a difference between being aware of it and trying to resolve it.
                                                  iv.     Maybe we can ask Kara to come for a couple of weeks to figure out a way to message the process. 
1.      Let’s start transferring Ops into the second meeting of the month to focus on these cases. 
a.      Revisit this as an agenda item. 
b.      Let’s set a time limit on how long we are spending on discussions. 
c.      Come in with what you’ve already tried in a way that is strengths based.

a.      Shelter Waitlist Process Questions – Stephanie Corbin
                                                    i.     Central CAN Transfer Request - GH CAN approved Stephanie to add sex offenders from CCAN who could not be diverted to the Shelter Priority List for GH CAN.

                                 c.      Outreach Worker List Updates – Alexis Gaynor - GH CAN Approved new official designated outreach workers who can call shelters to immediately identify shelter space.


6.      Announcements
a.      Family Shelter Check-In will be taking place immediately after today’s meeting from 3:30-4:00PM
b.      Data around housing and our shelter priority list is available on p. 2
c.      CCEH has funding available for victims of Hurricane Maria- if you are working with anyone who was a hurricane evacuee please contact Mollie Greenwood or Lisa Quach immediately.
d.      CCEH has funding available for childcare for families in shelter!
e.      Rapid Exit Funding IS AVAILABLE.  Contact Mary.McGowan@journeyhomect.org with any questions!

Households at Risk of Return to Homelessness – Policy Adopted Last Month
Case managers or agency representatives are expected to bring up households who may be at risk of returning to homelessness as soon as a housing issues is identified and will use the case conferencing template.  A detailed review of the at-risk housing situations will provide the community a chance to brainstorm solutions, and providers who have existing relationships with the households may be able to support them in maintaining their housing.  All households at risk of returning to homelessness will be brought up for case conferencing at the Individual or Family Solutions Meeting as soon as feasible with a goal of 60-90 days prior to their housing being terminated.  Staff are expected to follow up on recommendations from the GH CAN and update the CAN in a timely fashion.
