GH CAN Leadership Meeting 3/17/2021

 

GREATER HARTFORD COORDINATED ACCESS NETWORK MEETING AGENDA

Wednesday, March 17, 2021 via Zoom

 1PM – 2:15PM

 

1.       Welcome and Introductions – 5 minutes

2.       COVID – 19 Agency Updates and ESG Updates – 5 minutes

a.       Vaccinations

                                                               i.      EH Shelter – Wheeler Clinic came over the weekend to provide COVID test and provided education for vaccinations. Will follow up with residents to see if they are comfortable receiving vaccinations.

                                                             ii.      ImmaCare – no major updates. Continuing to talk to residents about getting vaccinated through Charter Oak.

                                                           iii.      SA – not tracking or requiring vaccinations. Some staff were hesitant but have changed their minds. Working with the city to provide residents with vaccinations. Waiting for the city to receive J&J vaccines for shelter residents.

                                                           iv.      HOB – vaccine clinic through Charter Oak health to be distributed tomorrow and next week.

                                                             v.      McKinney – next month will do a one time vaccination with HHC. Have completed vaccinations last week.

                                                           vi.      YWCA – 2nd round of shots will be next week. PSH residents qualify for vaccines Charter Oak will return next week to provide vaccines. Will include new shelter residents and PSH residents.

                                                          vii.      HOH – some staff and tenants have received vaccines, will receive 2nd dose soon. Looking to receive vaccines for day guest.

                                                        viii.      Cornerstone – some tenants are hesitant. Next round of shots will be next week.

                                                            ix.      CRT – hosting a vaccine clinic for those with high risk and meet eligibility requirements.

                                                             x.       Chrysalis – 2nd shot was today. InterCommunity provided vaccines. 75% of staff have received vaccines.

                                                            xi.      TOH – 100% participation in vaccination. Group of 12 clients/staff will be vaccinated soon.

                                                          xii.      GHHRC – almost all staff are vaccinated. Working to vaccinate the rest. Working with Jose.

                                                        xiii.      CHR Enfield – offering vaccine to staff and all shelter residents.

                                                        xiv.      Capitol Region – received 2 doses. Need to collect percentage of those vaccinated.

                                                          xv.      SPI – last of staff to receive 2nd dose of vaccinations.

3.       Cold Weather/Winter Planning Updates – Lisa Cretella, Bryan Flint, Kara Capone, Mark Jenkins, Emily Soucy, and Geri Harrison– 15 minutes

a.       Closing of Hotels, Warming Centers, and Triage

b.      CHR Enfield – trying to figure out next steps for those who need it.

c.       SA – reduced WC capacity to 40, planning on closing WC and overflow on 3/31. Families have a lot of barriers. LOS at overflow 19 days. Will keep all families at overflow till they move to permanent beds. Triage location has moved, will be at Mercy location for the last two weeks.

d.      Mercy – will close 3/31. Preparing to close soon, will not extend to April.

e.       Cornerstone – at highest capacity right now, meeting this morning with Town of Manchester- discussed what are the barriers, will close on 3/31.

f.        In communication with all hotels and WC, discharge plans and housing plans for all stayers. 11 folks who need extended hotel stays. Proposing 10 rooms at BW into April for those who need extended hotel stay – no including families. Mark’s team will stay an additional month. CAN has approved. Working with Diversion for messaging for those who are having extended stay. All shelter beds will be filled by outreach, will not use waitlist. Will track those who need shelter on outreach referral sheet. Process and protocols will be send to providers and 211.

4.       Updates/ Next Steps on Accessing In-home Intensive Services and Accessing Higher Level of Care/Residential Treatment Programs – Crane Cesario - 20 Minutes

5.       Review CAN ROI Removal Policy for Non- HMIS Users –Lisa Quach – 5 minutes

a.       Possibly two policies

b.      Agreement with rights and responsibilities

c.       Appeal process. Can be added to the draft. Continue conversation. If a threat is made –immediate termination!

d.      Warning – present a timeframe

6.       Family Homelessness and RAPs – Lisa Quach – 5 minutes

a.       March 24th at 3:30pm, Statewide Family Provider Meeting – CAN COVID Taskforce invitation from Danielle Hubley

b.      GH and Central CANs have been awarded 40 RAPs. Already matched 20+ RAPs.

7.       Balance of State Continuum of Care Context/Updates –10 Minutes- Crane Cesario

a.       COVID meetings on Friday to sign up, visit CTBOS website.

b.      Reallocation discussions – two notices to agencies

c.       Project priority – PSH programs

d.      Ongoing work for antiracism

e.       Allocation due in Summer, awards in Dec.

f.        Upcoming events on website

8.       Future Agenda Items or Additional Announcements – 1 minutes

a.       Prioritization for Rapid Rehousing

b.       Agenda item – Documentation Readiness for Referrals
